I acknowledge and agree that:

• I understand that I am required to have a Unique Student Identifier (USI) to undertake nationally recognised training. I consent to Paradigm Training Group verifying my USI with the USI Registrar via the aXcelerate system and understand that a Statement of Attainment or Qualification cannot be issued without a verified USI.
• I understand the course participation requirements, including completion of any online learning components, attendance at required face-to-face training (including the practical workshop where applicable), and successful completion of all assessment tasks.
• I consent to Paradigm Training Group using my personal information to manage my enrolment, deliver training and assessment, meet reporting obligations to relevant authorities (including DTET, NCVER, ASQA, and AMSA where applicable), and issue my qualification or Statement of Attainment.
• Where applicable, I consent to my enrolment details, attendance, and training outcomes being provided to my employer.
• I understand that photographs, video, or other recorded evidence may be collected during training and assessment for the purpose of demonstrating competency and meeting assessment requirements.
• I understand that compliance with Paradigm Training Group policies and procedures, including the Student Code of Conduct, is a condition of my ongoing enrolment, and that my enrolment may be suspended or cancelled if I breach these requirements or behave in a manner that endangers myself or others.

Where applicable, I acknowledge that:

• any government funding, subsidy, employer funding, or other funding arrangement relevant to my enrolment has been explained to me, including my eligibility, obligations, and any impact on fees, enrolment conditions, or future access to subsidised training;
• I have accessed and read the Qualification course information available on the Paradigm Training Group website, including the total co-contribution fee (concessional and non-concessional), how and when fees will be charged, delivery mode, training timelines, assessment methods, delivery locations, completion requirements, licensing outcomes (including AMSA where applicable), maritime pre-requisite certificate requirements, and available student support services;
• I have been provided with the co-contribution fee schedule for my chosen qualification and understand how and when fees will be charged;
• I understand that co-contribution fees are charged at the unit of competency level, and that a maximum of 30% of the total fee may be collected prior to training commencement, with the remaining balance invoiced at agreed progression points;
• I understand that co-contribution fees cannot be waived or paid by Paradigm Training Group, but may be paid on my behalf by my employer, a family member, or another third party;
• I have read and understood Paradigm Training Group’s Refund Policy, including entitlement to a full refund prior to training commencement, proportionate refunds where applicable after commencement, and conditions relating to employer contributions;
• I have been informed of my Career Start (or relevant funding program) entitlement and understand when that entitlement may be exhausted upon enrolment or completion of this course;
• I understand that, as a participant in subsidised training, I may be contacted by the Queensland Government to complete surveys (including Student Voice or similar) regarding my training goals, experience, and outcomes;
• I understand that if my eligibility changes, or if the information I have provided is false, incomplete, or misleading, this may affect my enrolment, fees, or funding status, and may result in cancellation of my enrolment and/or recovery of any government funding;
• I understand that eligibility requirements for government funding may change, and I agree to notify Paradigm Training Group of any change in my circumstances that may affect my eligibility; and
• I understand that I must complete and submit this enrolment within 30 days of my Pre-Enrolment Assessment (AISS) with Paradigm Training Group, otherwise eligibility may need to be reassessed prior to enrolment proceeding.

I acknowledge that my personal information is collected by Paradigm Training Group (RTO #41059) in accordance with the Further Education and Training Act 2014 (Qld), the Student Identifiers Act 2014 (Cth), and the Australian Skills Quality Authority Act 2011 (Cth).

I understand that my information will be used to manage my enrolment and training, monitor my progress, fulfil legal and regulatory reporting obligations, and issue qualifications and Statements of Attainment.

I understand that my information may be disclosed to relevant authorities and organisations where required, including the Department of Trade, Employment and Training (DTET) for funding, compliance reporting, and student surveys; the National Centre for Vocational Education Research (NCVER) for national VET reporting; the Australian Skills Quality Authority (ASQA); the USI Registrar; and the Australian Maritime Safety Authority (AMSA) where licensing outcomes apply.

I understand that Paradigm Training Group will only release information relating to my enrolment, attendance, progress, results, or participation to any other third party where my prior verbal or written consent has been obtained.

I understand that:

• no information will be released to a third party without my consent;
• consent may be provided verbally or in writing and may be recorded;
• I may withdraw or restrict this consent at any time in writing; and
• any disclosure will be limited to information relevant to the authorised purpose.

I acknowledge that I have been informed of the potential use of photographs, video, recordings, and testimonials for training, assessment evidence, and promotional purposes.

I consent to being photographed, filmed, recorded, or quoted for training, assessment, marketing, website, social media, and testimonial purposes.

I understand that I may withdraw this consent at any time in writing and that refusal or withdrawal of consent will not affect my enrolment or participation.

By accepting these terms and submitting this enrolment, I acknowledge that:

• my acceptance constitutes my electronic signature;
• this agreement is legally binding;
• the date and time of submission will be recorded; and
• this record will be retained in my student file as evidence of my declaration, consent, and acceptance.
